New Physical Form
From The Activities Office

Effective January 1, 2010, Public Act 096-0128 became law which requires that the pre-participation physical form shall include a question asking whether a student has a family history of cancer and may require a testicular examination as part of a physical required for a male student's participation in interscholastic activities. The pre-participation physical form or card that has been used for many years does not include the question about a family history of cancer and therefore, that form is now obsolete and should no longer be used. Also, there is a section on the form where the students and parents give consent to random testing for steroids and performance-enhancing substances. This pertains only to high school students and is required by the IHSA for participation.

You may access the new physical form by clicking the link below or navigating to the forms page of this website.
